Ownership Change Form

Both the current owner and future owner should print the form below on their company letterhead. Complete the form and return to Aspire no later than the 10th of the month prior to the production month in which the change will become effective. Please make sure that the current and future owner information as well as the meter information is the same on each copy provided.

Do not send copies of the Assignment or State of Ohio Form 7 as they do not provide the necessary information.
